The footballer was left out of the squad for the Toffees' Thursday night Carabao Cup fourth-round tie against Newcastle due to a breach of club discipline.
The Press Association understands the breach relates to an unauthorised absence from training.
The 31-year-old also missed Saturday’s 2-0 loss to Arsenal, but that was put down to illness.

Grealish, who is on a second consecutive season-long loan from Manchester City, had started Everton’s previous three Premier League matches against Ipswich, Hull and Chelsea, and has made a total of six appearances for the Toffees this term.
Last season, Grealish provided six assists and added two goals in 20 appearances before suffering a season-ending foot injury in January.
Having enjoyed his time on Merseyside, Grealish remained with Everton during his rehabilitation, and returned on loan on September 1.
This development came after Grealish was spotted out and about in the Cotswolds recently, with a trip to a restaurant leading to a photograph with the staff.
The England international dropped in to Pan-Asian eatery You, Me & Umami in Lechlade’s High Street, not far from Swindon and Cirencester, to pick up a takeaway.
You, Me & Umami serves a menu of bold flavours and pan-Asian favourites, from bao buns and gyoza to ramen, Thai curries and Indonesian nasi goreng.
A spokesperson for the restaurant said Grealish’s visit gave owner Foyez Rahman ‘a celebrity encounter to savour’.
“It was a real surprise to welcome Jack to You, Me & Umami,” Mr Rahman said.
“You never know who might walk through the door, and it was fantastic to have him visit us here in Lechlade.
“We’re proud of what we’re building and love seeing people from all walks of life enjoying our food.”
