Roma started strongly and took the lead in the 20th minute through Marten de Roon, who scored with an effort from outside the penalty area.
Real Madrid responded five minutes later when Kylian Mbappe scored from distance following an assist from Yan Diomande to make it 1-1.
However, Roma regained control after the break, with Donyell Malen scoring in the 51st and 57th minutes to give the Italian side a 3-1 advantage.
With Roma appearing close to victory, Jose Mourinho’s Real Madrid increased their attacking pressure in the closing stages. Carlos Espi scored in the 83rd minute to reduce the deficit, with VAR confirming the goal after an initial offside decision.
Endrick then brought Madrid level in the 85th minute, heading home from an Arda Guler corner to give the visitors renewed hope.
The comeback was completed in dramatic fashion in the 90th minute when Antonio Rudiger reacted quickly to a loose ball and scored from close range following another corner delivered by Guler.
Roma pushed forward in search of an equaliser, but Madrid held firm to secure all three points after an exciting encounter.
The victory moves Real Madrid up to eighth position in the Champions League standings, while Roma drop to 25th after failing to protect their two-goal advantage.
The result will provide Madrid with a major confidence boost as they continue their European campaign, while Roma will regret missing the opportunity to secure a valuable victory at home.
